Advanced Nurse Practitioner (or *Trainee)
Up to 37.5 hours
*Salary: Band 7 Annex 21 for Trainee ANP
We are seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations. Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions and to lead sensitive discussions around anticipatory care planning and end of life care. As part of integration the role will involve working across organisational boundaries and in partnership with other health and social care teams.
The successful candidate will join a team who focus on the assessment and treatment of our frail and older patients presenting with any acute deterioration in our care homes and intermediate care settings over 7 days.
We may also consider applications for trainee posts and will support the development of individuals to meet advanced practice competencies using a triangulation approach of academic courses, professional supervision and clinical practice supervision. Applicants for trainee posts should have demonstrated evidence of undertaking courses relevant to the advanced practice pathway.
Essential Qualifications for band 7:
- HEI accredited Independent Prescribing Qualification - V300
- HEI accredited Clinical Examination & history taking course
- HEI accredited clinical decision-making module
Highly Desirable
- Previous advanced practice experience
- Experience in autonomous practice in primary care such as home visits or care homes

- Post graduate diploma in advanced practice
