An exciting opportunity has arisen to join our Advanced Nurse Practitioner team within Dundee Health & Social Care Partnership initially as part of our Urgent Home Visiting Team, but with a view to working across our developing community urgent/enhanced care services.
Applicants will require experience working at an advanced level with the ability to make professional autonomous clinical decisions through history taking, physical examination and problem solving. The role will involve working autonomously supported by the Lead Advanced Nurse Practitioner and working collaboratively with all members of the multidisciplinary team within community settings.
You will pro-actively manage and review adults with acute care needs living in care homes or community settings in Dundee, you will be expected to act as a clinical leader, working in close collaboration with the members of the MDT.
You will be expected to act autonomously in the assessment, diagnosis, treatment, including prescribing for adults who have acute medical needs. You will work closely with people and their families/carers and care home staff to identify clinical issues quickly and aim to support avoidable deterioration, admission/readmission to inpatient settings.
You will also work with the Discharge Hub to facilitate timely discharge and smooth, efficient and person centred transitions of care.
You will play a key role in the leadership and development of advanced nursing practice within Urgent Care, will participate in the delivery of specialist education and training programmes and work collaboratively with health and social care colleagues across the partnership to deliver high quality evidence based care.
You will also play a key role in enhancing patient experience, assuring clinical governance and supporting the Lead Advanced Nurse Practitioner and Senior Leadership Team in the improvement and continued development of the service.

Applicants will be registered nurses and must hold a post graduate diploma, be willing to work towards this or hold a masters degree in Advanced Practice. You must have the personal drive, clinical knowledge and commitment to undertake this challenging and rewarding role. The ability to travel across Dundee is essential. You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register
