An exciting opportunity has arisen for a Salaried GP to join the team working within The Abbey Practice, Arbroath. We are an NHS board run practice with a list size of 6800 patients approx.

You will work with the Practice Clinical Team and Practice Manager and will be supported by the Lead GP for The Abbey Practice.

You will be an integral part of the practice team, with the opportunity for close working with colleagues from both health and social care. This will include named consultant lead teams in psychiatry, medicine for the elderly and palliative care working across the Locality.  You will have the opportunity to contribute to the development of the range of services offered, whilst exploring new models of care to fit the needs of changing demographics.

We will be looking to support new models of working and continue to work closely with community resources including community pharmacy, community nursing and local voluntary sector. We have weekly MDT meetings with community nursing, medicine for the elderly team, community pharmacist and social care teams to help pro-actively plan and manage care for frail elderly patients.
