Carden Medical Centre has a list size of around 9000 patients. The team you will be working with at Carden consists of, 5 Salaried GPs, a Nurse Practitioner and a very experienced Practice Nursing team - which includes a Health Care Assistant and Nurse Prescriber.
The Practice uses Vision, has an excellent Admin Team and works from purpose-built premises which they share with other healthcare professionals including District Nurses, Health Visitors, Midwife, Link Worker, Psychological Therapist, Social Work and Occupational Therapy staff. Our Practice offers extended opening hours.
Soon to be a Section 2c Practice, staff will be employed by NHS Grampian and managed by Aberdeen Health and Social Care Partnership (ACHSCP). We are looking for a motivated, enthusiastic and hardworking Lead GP to clinically lead the dedicated team at Carden Medical Centre to provide high quality care to our patients in an environment of warmth and understanding.
We are looking for a dynamic individual with a vision for change and leadership that is able to guide and shape the developments and tests of change in Carden and share the learning across all City 2C Practices with the support of the Primary Care Team and Aberdeen Health and Social Care Partnership Leadership Team
This role will work in collaboration with other Clinical Leads across the city, Heads of Locality, Professional Leads and other Operational Managers for Primary and Community Care, to ensure a coherent whole system approach to the delivery of services both within the specific locality and across Aberdeen City.
Aberdeen City Health and Social Care Partnership covers the geographical area of Aberdeen City which has a population of approximately 235,000. Within Aberdeen there are currently 29 GP Practices. The City is grouped into four localities based around groups of GP Practices.
The successful candidate(s) will have overall strategic and operational responsibility for the management and delivery of General Medical Service (GMS) services for Carden Medical Practice
The practice operates under the General Medical Service (GMS) contract for General Practice, the Strategic Plan for Aberdeen City Health and Social Care Partnership, the National Health and Wellbeing Outcomes and other relevant professionals to ensure the implementation, development and delivery of safe and effective, person-centred systems to enable maximum operational efficiency across Aberdeen.
The Aberdeen City Health and Social Care Partnership is responsible for the planning and delivery of Adult Health and Social Care Services in Aberdeen City as well as hosting Older People and Rehabilitation Hospital based services at Woodend Hospital and Sexual Health Services based at the Community Health and Care Village on behalf of Aberdeenshire and Moray Integrated Joint Boards.
