We are looking for a motivated, experienced and driven Physiotherapist to join the Boroughloch Medical Practice MDT. Applicants should have an interest in the management of Long Term Conditions, Anticipatory Care Planning and Multidiscplinary working. They will work alongside another Advanced Physiotherapy Practitioner care managing community and Care Home patients that are part of the Boroughloch Medical Practice. They will be supported in this role by the lead GP and the wider MDT. We are looking for someone to work the Mon/Tues/Wed.
To undertake the role of an advanced physiotherapy practitioner, utilising high levels of decision making and clinical judgment. Ensure that patient health needs are met through the provision of the diagnosis and the management of patient conditions through the assessment, treatment, review and planning process in partnership with patients and families, and the Primary Health Care team. Provide specialist physiotherapy advice to healthcare professionals, other agencies, carers, clients and relatives. The AP physiotherapy role encompasses that of expert clinician, educator and auditor. Take a key role in leading and developing the service to improve practice and work collaboratively, implementing and evaluating evidence based standards, guidelines and policies. To identify, develop, co-ordinate and deliver appropriate research activity which will contribute to the further development of the service. This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
