An exciting opportunity has arisen within Forth Valley Royal Hospital, based at Larbert. This Frailty Project Team Leader post is an exciting opportunity to develop and improve the journey of Frail patients who attend the acute receiving areas of the hospital.
This post would be ideally suited to an experienced, highly motivated and enthusiastic nurse, with previous experience at Senior Charge Nurse/Team Leader level. The successful candidate must have an interest in frailty patients with acute medical issues and be able to manage the complexities attached to patients’ care including effective discharge planning skills.
As part of a multidisciplinary team, including AHP staff, psychiatric liaison, consultants, discharge coordinators, community teams and integrated social services the post holder will have responsibility for developing a team and ensuring adequate training to facilitate screening, assessment, and coordination of the patients identified as Frail to expedite early intervention and planning.
The successful applicant will have excellent communication skills and have an extensive networking knowledge of staff and services related to acute elderly care. Effective computer skills are also essential to capture and collate data figures.
A genuine interest and clinical knowledge of elderly care medicine is essential. Ward management experience is desirable and you should demonstrate leadership abilities within a ward team .You will be expected to work a flexible work pattern.
