The shift pattern for this post is 8.30-4.30 pm Monday-Friday.
An opportunity has arisen for Band 5 community staff nurses educated to degree level to work within School Nursing who will be responsible and accountable for the assessment, planning and implementation of programmes of care as directed by the qualified School Nurse( ScPhn).
Programmes of care will be developed in partnership with children, young people, families and others as appropriate. Programmes will be evidence based taking into consideration lifestyle, race, gender, cultural background and additional factors which may impact on physical, mental or emotional health. Programmes of care will adhere to the current pathways relating to Child Protection, Emotional health and wellbeing & Transitions
Working in partnership with colleagues, children, young people and families, education, the local community, Health Improvement colleagues and other agencies to identify, address and implement approaches to support health needs and improve outcomes for children and young people.
