Older People and Stroke Service in RAH is offering an exciting opportunity to be part of a multidisciplinary team with responsibility for delivering Comprehensive Geriatric Assessment (CGA) to older adults on admission to RAH. The role will facilitate CGA and provide Liaison throughout the hospital. This role will support safe onward transfer and supported discharge of patients through appropriate screening and patient/family/carer involvement.
The post holders will act as an expert resource providing advice where required to members of wider multidisciplinary team. This role will incorporate audit to measure impact of service and identify ongoing areas for improvement. As part of wider team you will contribute to strategic issues both locally and nationally.
