37.5 hours, 18 months Fixed Term or Secondment

Do you have excellent communication skills, an ability to work in partnership with
others, a patient focused approach and a desire to provide and improve services to meet the needs of patients with diabetes?

Then come and join our Community Dietetic Team in Grampian where you will work to scope, develop, deliver and evaluate an Adult Weight Management (AWM) service in Primary Care for patients with diabetes.

We are keen to look at new and transformational ways of delivering adult weight management services and the post holder will undertake the following over an 18 month period.

•    To take a quality improvement approach to develop, deliver and evaluate a structured programme for people with diabetes
•    To determine the need for remote structured group education versus in-person group education programmes
•    Work in partnership with other staff and agencies in the locality areas to ensure good quality nutritional care
•    To hold a patient caseload, carrying out dietary assessment, implementing treatment packages and providing ongoing review and monitoring

You will be part of a team of community dietitians within Aberdeen City and Aberdeenshire but will also work as part of the wider health and social care team to deliver services across the partnerships.  This post is suitable for a hybrid model of working with options for both remote and office based working.

This is an ideal opportunity for candidates wishing to develop and progress their career and professional practice in a department that recognises the value of ongoing CPD, robust clinical supervision system and exposure to a variety of development opportunities and for candidates seeking a secondment opportunity.

We participate in undergraduate student training, taking students for their B & C placement. We work with dietetic colleagues across Grampian on shared issues and initiatives such as clinical effectiveness, patient safety and patient centeredness.
Delivery of a high quality dietetic service is high on our agenda.

You must be a car driver/access to car and hold a full driving licence.

You will be required to work 5 of 7 days which may include weekend working as required
