NHS Borders are delighted to invite applicants to join our Mental Health Older Adults Service- Care Home and Community Hospital assessment team (MHOAS- CHAT) as a Band 6 Liaison Nurse
MHOAS- CHAT aim to deliver person centred, goal orientated and strength based care for individuals currently in 24 hour care settings. Your role would include Mental heath assessment, Diagnosis, treatment and supporting the team to provide support to individuals within the Scottish Borders.
Contributing in MDT discussion, case load discussion and management, theraputic interventions for individuals, Collaborating with other services to support staff and provide training, joint working and information sharing to benefit individuals
We are seeking a highly motivated Liaison Nurse with experience, knowledge, participating in ongoing learning and development for self and team, leadership skills, supporting staff development within team and good communication skills.
We are a close, supportive and inclusive team with high standards for delivery of clinical services within the Scottish Borders
