Community Link Workers already play a key role as part of Annandale & Eskdale’s Health Improvement Team and we have some exciting new opportunities in order to expand the team to meet the needs of the new Scottish GP contract.
Community Link Workers offer person centred support to enable individuals in the community to make positive choices and changes that will help to improve their health and wellbeing and quality of life. This approach builds on strengthening the knowledge, skills, social connections and resilience of individuals and communities to improve health and wellbeing and work towards reducing health inequalities.
The post holder will also work as part of a team to support the development of community health and wellbeing activity based at SONAS which is located within the grounds of Annan Hospital.
We are seeking candidates who have relevant qualifications and experience, as well as excellent communication and inter-personal skills; they will also be required to work well as part of a team and with a degree of autonomy as needed.
Applicants should be able to travel independently and have good IT skills.
