An exciting opportunity has arisen for a Band 3 Health and Social Care Support Worker to join the team and work as part of the Islay and Jura Community Team.
The role will involve assisting staff in the delivery of interventions, care and support, which will allow individuals to remain in their own homes and to facilitate early discharge from hospital. The post holder will work with patients/clients in hospital and within their own community setting in relation to identified care plans and goal centred activity, focussing on the key priority areas of maximising independence and supporting the individual to remain in their own home.
Travel will be required to meet the identified needs of patients / service users and this will be dependent upon location and caseload.
The post holder will be required to work flexibly to cover services on a rota system worked over 7 days a week and including unsocial hours.
