We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included. 

The post is 5 days over 7 but predominately Monday to Friday. The team provides a seven day service over the winter months and a public holiday service to the acute assessment wards at GRI.

The AHP front door and Older Peoples Service are looking to recruit a Band 7 Occupational Therapist or Physiotherapist. This is an exciting new opportunity as the post holder will play a key role in the development and implementation of an Integrated Frailty Service within North Sector. You will provide expert assessment, clinical care and advanced management for the frail older person who presents at the acute front door.  The post will be predominately within the emergency department, assessment unit and assessment wards within Glasgow Royal Infirmary. Where clinically appropriate, you will facilitate early hospital discharge by working with the hospital-based teams to assess, plan and deliver care for patients who are vulnerable, have frailty and experience complex multi- factorial health and social issues. The post holder will undertake comprehensive geriatric assessment as part of the acute pathway, providing effective interface with health & social care partnerships ensuring timely discharge to homely settings.

You will work collaboratively and provide peer support and advice to colleagues across Health and Social Care, actively promoting and collaboratively contributing to community alternatives to admission and support for discharge.  

The North Sector has been accepted on the HIS ( Health Improvement Scotland ), Focus on Frailty Program and this is an excellent opportunity for the successful candidate to be involved in the planning and development of this service, The post holder will also be able to access education and training opportunities through the GGC Frailty Team within GGC.

Essential

  • Diploma/Degree in Occupational Therapy or physiotherapy
  • Registered with the HCPC.
  • Must hold a practice education qualification.
  • Up to date mandatory and statutory training record e.g. Fire, Moving and Handling.
  • Proven specialist skills and knowledge in relation to OT or PT  in Older People’s Service/Door  gained through experience of working within the specialty and experience gained through working in multi-disciplinary or multi-agency teams.
  • Working towards masters level or equivalent experience
