The shift pattern for this position is 5 days -mainly Monday - Friday.

As a Diabetes Clinical Nurse Specialist, you will be a registered nurse who has acquired significant Diabetes specialised knowledge & have current experience in the management of Diabetes clinical care. Preferably you will have previous experience as a Clinical Nurse Specialist and have the knowledge and skill set to competently perform the necessary technical/invasive/physical aspects of care to support patient compliance with treatment /management of Diabetes.

You will act as a resource for expert advice and training, for nursing staff, junior medical staff and other healthcare Professionals whilst facilitating an immediate point of contact for urgent Diabetes patient consultation. You will plan efficient inpatient care & independent follow up out patient reviews as necessary to facilitate health , wellbeing , whilst assisting with prompt discharge and admission avoidance.

You will be responsible for consultancy, teaching, audit, research, participate in service development in conjunction with the Diabetes Team Lead /Team and maintain service objectives to meet the changing demands of delivering specialised Diabetes care in the South sector. You must have the ability to work autonomously and possess a high level of interpersonal and confident communication skills to manage & support this specific client group.

You will make arrangements for follow up care and will be responsible to engage with various teams within Primary & Secondary care including hospital clinicians, Dieticians and General Practitioners & maintain excellent clinical links between the services, whilst upholding confidentiality at all times.

You will be expected to contribute professionally within groups which may cross-traditional or service boundaries and possess a high level of ITC skills including e- forms, Trak care and portal.

You will be based within the South Sector and shift pattern is predominantly Monday - Friday as the service requirements dictate. It is essential that you are currently a NMC Registered Adult Nurse.
