Within the Health & Community Care Directorate, the Primary Care Team are responsible for the direct delivery of Primary Care Transformation through key programmes of work including Primary Care Improvement Plan, Unscheduled Care and other relevant initiatives. The team, leads on service planning and development, supporting delivery of improvement activities under the direction of the Primary Care Transformation Manager.

The post–holder will play a pivotal role in supporting and driving forward:

  • Service improvement activities across a range of projects at any one time; within defined timescales to meet required outcomes.
  • Programmes of work across Primary Care services to promote and facilitate the development of interagency and multidisciplinary working.
  • Planning, implementing as well as adjusting and delivering on project and improvement activity on a day-to-day basis.
  • Information management, data analysis for reports including the design and development of programmes of data and spreadsheets for the purpose of local audits, surveys and monitoring.

The post holder will:

  • Co-ordinate specific service user and workforce engagement; supporting the implementation of improvement Engagement and Communication Plans.
  • Create and maintain improvement work plans and action trackers to prioritise workload to ensure achieved outcomes within agreed timescales.
  • Assume an active and participative role in, groups associated with Primary Care Transformation, specifically in terms of planning and performance, and undertake any required training.
  • Work directly with managers within a range of seniority to promote, support and facilitate effective working relationships.
  • Work autonomously to plan, manage & deliver multiple improvement work streams.
