This post is fixed term for 18 months.
This new opportunity has arisen from funding to deliver a key priority of the Heart Disease Action Plan (2021-2026) which is to improve timely access to investigation, clinical care and psychological support for families in Scotland affected by sudden unexpected death (SUD), sudden cardiac death (SCD) and out of hospital cardiac arrest (OOHCA). This post will be hosted by the West of Scotland Inherited Cardiac Conditions (ICC) service and will deliver educational and audit work nationally through close involvement with the Network for Inherited Cardiac Conditions Scotland (NICCS). Patients and families have very different experiences when dealing with a SUD/SCD/OOHCA. The appointed person will work across organisational boundaries and professional teams to pilot an integrated care system that is based on individual and family needs. As a clinical leader the CNS will be able to act autonomously in the assessment, diagnosis, treatment and referral of patients with or at risk of heart disease. This post will be fully integrated into the West of Scotland ICC multi-disciplinary team. The CNS will lead co-ordination and delivery of the clinical referral pathway and gather evidence from patients and professionals to inform development of a national SUD/OOHCA coordinator role. During this project, the CNS will contribute to development of information on public platforms and online resources.
