An exciting opportunity has arisen for an experienced healthcare professional to join the South Perthshire & Kinross Locality Integrated Care Service (LINCS). LINCS is a responsive and coordinated multidisciplinary care approach for patients over (18+) living in Perth and Kinross, who are assessed as being medically safe to be cared for at home. The support is intended to be a timely and short escalation of care during which time a person’s needs are managed by the Clinical Coordinator in conjunction with the key clinicians and support staff within the team. The main aims are to improve patient experience and outcomes whilst reducing the need for unnecessary emergency admission or readmission to hospital or long term care.
The post holder will be responsible for the clinical triage of all referrals to LINCS on their designated working day, ensuring an appropriate care pathway is established for each individual. This requires advanced clinical knowledge and skills to holistically assess those with complex care needs, excellent communication and leadership skills, and the ability to work confidently and proactively as part of an integrated multidisciplinary care delivery team.
This will include:
- Screening all referrals and coordinating support for people referred to the Locality Integrated Care Service.
- Responsibility for the clinical triage and potentially the initial clinical assessment, coordination and mobilising support from the appropriate clinical and social work teams to provide urgent and timely proactive support to those requiring care within their locality.
- Identification of a range of early interventions, including promoting personalised self care planning, for those who have a deterioration in their condition or function in order to prevent further deterioration.
- Planning and implementing a wide range of clinical and environmental assessments to support patients to remain at home, or in a homely setting, preventing unnecessary hospital admission where possible.
- Supporting discharge and coordinating transition back home for patients with complex needs to optimise function and independence and prevent unnecessary readmission to hospital.
- Anticipatory Care Planning, ensuring links with general medical and other specialist services.
- Proactively communicating with patients, families and carers on the treatment and support plans and reporting back to referrers and others.
We are looking for an individual who has experience of clinical assessment and caring for people with complex care needs. You will have advanced verbal and non-verbal communication skills. The ability to work well under pressure and respond positively to unpredictable situations is essential.
We are committed to ensuring staff feel supported with their wellbeing and development through supervision, appraisal and training. Staff are expected to be part of a variety of Quality Improvement initiatives.
You must be a First Level UK Registered General Nurse or AHP equivalent with previous post registration experience, with current UK NMC or HCPC registration.

Hours of work are: 7.5 hours. Working day is negotiable.
