An exciting opportunity has arisen for an experienced and ambitious Senior Project Manager. The role will support various change initiatives across Fife Health and Social Care Partnership, but specifically the ‘Home First’ programme within the Community Care Services Portfolio.

The Home First Programme is directed by the recently published Home First Strategy, which can be found in the following link 20230922-home-first-strategy.pdf (fifehealthandsocialcare.org).

The vision of the Home First Programme is to enable people in Fife to live longer healthier lives at home or in a homely setting. The focus of the work is to develop the future model of community care in an integrated manner, with an emphasis on prevention, anticipation and supported self management to realise this vision. The programme outcomes will ensure that people get back into their home or community environment as soon as appropriate, with minimal risk of re-admission to hospital. Services will be redesigned/developed so they are flexible to growing and changing demands, as well as being sustainable.

The post will involve working collaboratively with multi-disciplinary colleagues and stakeholders to develop and operationalise implementation plans. You will have a proven track record in delivering complex projects within tight deadlines, preferably within the Health and Social Care sector.
