An exciting opportunity has arisen within Caithness Integrated Teams for a Health and Social Care Coordinator (HSCC).
The HSCC plays an integral role within the Integrated Team, gathering referral details from various sources including clients, their family members and other Health and Social Care Professionals. This information is then shared with the multi-disciplinary team in order to prioritise and allocate.
The HSCC provides support to professional staff when dealing with urgent and critical cases in the community.
The successful candidate must have excellent communication skills, good time management and organisational skills and have a flexible approach to their work.
A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes and be willing to travel effectively and efficiently between locations.
This is a fixed term full-time post of 37.5 hours to July 2024 to be worked in accordance with service requirements.
