An opportunity has arisen for an individual to join the North Perthshire Older Peoples Community Mental Health Team as an Assistant Practitioner.
The successful applicant will have experience of working with people with a wide range of mental health problems. Clinically, you will work in partnership with people to provide psychosocial and psychological interventions, facilitating, enabling and supporting them within their homes and communities. Within your role, you will support the CMH Nurses to ensure the delivery of safe, effective, person-centred care.
The post holder will be responsible for the assessment, formulation, implementation and evaluation of programmes of care that proactively involves the person, their families and carers and optimises self-management. The post-holder will require to work without direct supervision on a regular basis, referring to the registered nurse as required.
The applicant should have an SVQ- level 3 or equivalent.
This post will involve travel across North Perthshire.
The shift pattern for this post is worked Monday to Friday 08:30 – 16:30, but with the ability to work flexibly when required.
The base is Pitlochry Community Hospital, Pitlochry
