The NHS Fife Specialist Palliative Care (SPC) service is looking for a forward thinking Clinical Nurse Specialist to work within the SPC Outreach service. The outreach service consists of the community, acute and day services. You will be part of a team delivering and supporting holistic care to the people of Fife with advanced life limiting illness. Through a treat and advise, educate and develop delivery model, we aim to meet the Scottish Governments Strategic Framework for Action recommendations that “By 2021, everyone who needs palliative care will have access to it”.
We have recently developed a single point of access to the service, enabling immediate specialist advice for all, and you will participate in the daily huddle and weekly board round. You will be a source of professional advice and expertise to colleagues within the Health & Social Care Partnership and the Acute hospital. You will assess, treat, prescribe and direct further management which may include sign posting, referral to other care pathways, supporting discharges from inpatient to community setting or liaising with colleagues when admissions to a variety of settings are required. You will work as part of a multi disciplinary team and we encourage further development both on an individual and at a service level.
