The Role

As part of Operation Flow Frailty Work Stream, NHS Lanarkshire is developing a Frailty Team that will work at the interface across each of the three Acute Sites (UHW, UHM & UHH). Comprising of an Advanced Practitioner, Advanced Practitioner AHP and Generic Assistant Practitioner, the team will have the flexibility to undertake tests of change across localities, Flow Navigation and the Emergency Departments. They will explore how best to influence patients’ journeys and produce the best outcomes.

The team will support flow, and manage frailty demands across the whole system by providing in reach to front door entry points to the hospital with opportunities for cross site communication and management of demand.  Aims of the frailty interface team will include:

  • Admission avoidance
  • Reduce demand for acute beds
  • Early identification of frailty in older adults
  • Reduce length of in-patient stay by utilising appropriate discharge pathways
  • Increase flow to frailty assessment areas


Job Purpose

The post holder will be aligned to the frailty clinical network and will work as an expert clinical practitioner responsible for providing expert clinical care and management of both the patient and his/her family, making complex, autonomous decisions using advanced skills and knowledge. They will have responsibility for the assessment of frailty patients and the delivery of complex transitions of care across a variety of settings. The post holder will be an experienced practitioner, who will act within their professional boundaries and provide expert advice and opinion to multidisciplinary teams (clinical and non-clinical) across acute and community settings within both the health and social care partnerships and other agencies. The practitioner must demonstrate competence within all four pillars of advanced practice and make professional, autonomous decisions for which he/she is accountable.

  • To commence Comprehensive Geriatric Assessment (CGA) at the earliest opportunity including functional, cognitive and social assessment from point of presentation to the system
  • To liaise with family, carers and other agencies to ensure assessment and care is provided in the best possible setting and alternatives to hospital based care such as outpatient care is utilised
  • To work alongside existing MDT teams, to provide expert advice and recommended solutions regarding the different options to support frailty positive patients and their timely discharge from hospital.
  • To provide specialist frailty education and training, promoting the home first approach to support early discharge from hospital
  • Use quality improvement methodology to progress service improvements and evaluate the impact on the length of in-patient stay and acute flow

What we'll need you to bring

  • Registered AHP with valid HCPC Registration.
  • Educated to/working towards/operating at Masters Level.
  • Post Graduate Diploma in Advanced clinical practice and expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently. 
  • Comprehensive knowledge of clinical guidelines and standards within the speciality.
  • Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality.
  • Evidence of application of advanced practice skills in speciality and applying clinical judgement.
  • Evidence of further developed knowledge and experience in management and/or leadership
  • Valid driving licence

Fixed Term or Secondment for 1 year

37.5 hours

Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post. 


Location and Working Pattern

This role will be based in the Frailty Team within one of our 3 Acute Sites (UHW, UHM or UHH).

The working pattern for this role is 5/7.
