Fixed term or Secondment Opportunity* for 12 months to cover substantive post holders maternity leave.

An exciting opportunity has arisen for a highly motivated Advanced Nurse Practitioner with a passion to improve the experience of our ageing and frail population to join the Hospital at Home Team in NHS Forth Valley.

The post holder must have knowledge and prior experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations.  Excellent communication skills are essential to lead sensitive discussions around anticipatory care planning and end of life care with patients and their families/carers’.

As part of the multidisciplinary team the Advanced Nurse Practitioner will be an autonomous practitioner, responsible for the assessment, diagnosis and timely intervention required to enhance the patients’ journey during acute illness which will, if possible, result in an efficient and successful discharge.

The post holder will have a full induction and orientation programme as well as ongoing support to assist in continued professional development. 

This post will also be considered as in training position under Annex 21 of the Agenda for Change terms & conditions.

The post holder will be required to work over 7 days per week including public holidays.

Ability and willingness to travel throughout the area of service delivery is essential for this post. Please note service delivery is Forth Valley Wide.

Essential Qualifications for Band 7:

  • HEI accredited Clinical examination & history taking course
  • HEI accredited Independent Prescribing Qualification – V300
  • HEI accredited clinical decision-making module

Highly Desirable:

  • Previous advanced practice experience
  • Previous experience working in a hospital at home team
  • Previous experience in autonomous practice in a community setting

Applicants should have or be working towards a post graduate diploma in advanced practice.
