The post holder will be based within the Community Mental Health Team for Older Adults (CMHTOA) in Huts 3 & 4, Falkirk Community Hospital.

As part of the Older Adult Community Mental Health Team the post holder will provide specialist mental health assessments and reviews of people who are mainly over 65 years old and living in the community, using appropriate assessments, implementing evidence based clinical interventions and promoting mental health awareness and well-being within the wider community.

The post holder has responsibility for managing their own community- based caseload for people with mental health needs and providing clinical support to the CPN staff nurse and HCSW.

We are seeking a dynamic and forward thinking nurse. Experience working in older adult is essential. Experience working within a community mental health setting would be advantageous.

The post holder will work across the Falkirk Health and Social Care Partnership to provide a comprehensive, multi-disciplinary community mental health service to patients and carers in partnership with other agencies.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
