An exciting opportunity has emerged for a band 6 Nurse to work with both the Respiratory Nurse Specialist and the Hospital at Home service.  Your primary responsibility will be providing comprehensive holistic assessments of patients with chronic respiratory diseases.

Our vision will focus on admission avoidance and early facilitated discharge of patients with complex conditions, supporting patients at home prioritising safety and professionalism grounded in evidence based practice. 

We are seeking experienced nurses who will embrace this dynamic opportunity. The post holder should possess knowledge of the management of patients with acute and chronic respiratory disease and will be required to demonstrate the ability to work autonomously, and as part of a team.  This will involve working across professional and organisational boundaries in partnership with other acute care nursing and medical staff, allied health professionals and senior management teams.  The applicant should posses the ability to prioritise fluctuating workloads, remain focused, and be flexible in an extremely busy environment

The collaboration of these two services is continuously evolving so candidates will need to be adaptable to change and able to contribute to any new initiatives to improve patient care.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
