An exciting opportunity has arisen within the East Sutherland Health and Social Care Team.  We are looking for a motivated & enthusiastic individual who will be part of a multidisciplinary team working with patients in their own homes to provide holistic care. Your primary responsibility will be to assist with enabling people to maintain their independence at home and supporting people after they have left hospital.  The post holder needs to be flexible and able to use their own initiative.  The post-holder will provide high standards of patient care in support of the team to ensure delivery of a high quality service to the local community.


This is a part-time post, plus an on-call commitment, to be worked flexibly in order to meet service requirements.  A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes
