Midlothian is growing clinical capacity across Primary Care in fulfillment of the delivery of a pharmacotherapy service for General Practice.

An exciting opportunity has arisen to join our award-winning primary care team, which is fully engaged in the delivery of a range of pharmacotherapy activities. This includes medicine’s reconciliation and monitoring of high-risk medicine. You will also be an active independent prescriber or supported to do so. The successful post holder will work in general practices delivering services locally as well as centrally with our team in the medicine’s reconciliation hub.

We require experienced pharmacists to deliver a range of clinical pharmacy services including polypharmacy reviews and assessment of patient medicines compliance, with services delivered to patients living at home and in care homes around the county. You will be responsible for monitoring service delivery and providing reports to key stakeholders including Health and Social Care Partnership clinical leads where appropriate. The Midlothian Pharmacy service is a supportive and nurturing team fostering an environment that allows individuals to flourish. We promote leadership at all levels.

The Primary Care Pharmacy Service is enthusiastic about participating in practice-based research using quality improvement methodology
