Pharmacists, are you ready for a change or to try something new?
Wigtownshire Community Health and Social Care (CHSC) team is looking to recruit a motivated, proactive and enthusiastic pharmacist to work within our Pharmacy Hubs and GP practices. Our team of pharmacists, pharmacy technicians and pharmacy support workers make up the GP practice clinical pharmacy service, taking a leading role in medicines management and medication review as part of the primary care multidisciplinary team. It is a rewarding role, helping patients in Dumfries and Galloway to safely manage their medicines.

You will provide a clinical pharmacy service directly to patients. Day-to-day responsibilities might include: clinical assessment and review of patients, performing complex polypharmacy medication reviews with patients, dealing with medicines related queries, assessing patients ability to manage their medicines (including in the patient’s home), medicine reconciliation and prescribing of medication. Joint working with GP practice staff will be routine, as well as with the multidisciplinary teams within the local hospitals, Community Pharmacies and Home Teams.
