The Mental Health Discharge and Resettlement Team  (MH DART) provides a clinical discharge and resettlement service to patients admitted to any of the acute mental health wards across who are experiencing homelessness or are at risk of homelessness. The team also offers a rehabilitation service within South and North East Glasgow along with the Rehabilitation MDT and provides follow up to individuals discharged from rehabilitation services. as an integral member of the multi-disciplinary team you will have the responsibility for clinical nurse leadership, managing and supporting the ongoing team development to fulfil the key functions of the DART Team.  In particular you will be responsible for the safe and effective deployment and management of the nursing resource to ensure robust risk assessment/ risk management, implementation of complex programmes of care using a strength based recovery focused approach that proactively involves the person and optimises self-management across the service. 

You will be required to undertake caseload management and line management responsibilities of senior nursing staff with overall responsibility for monitoring nursing activity related to DART key performance indicators.  Has lead responsibility for profession specific clinical governance activity and monitoring which includes care assurance; standards compliance; professional regulation; responding to complaints; staff governance; Clinical Supervision; Line Management Supervision; legislative frameworks; national and board policy/ guidelines  implementation.    This involves working in a variety of home/ near to home community environments.

Participating as a key member of multi-disciplinary decision making with regards to referral screening/ patient allocation and reviews as well as monitoring nursing workload (clinical activity) /workforce issues and ensuring resource efficiency are critical aspects of the role. 

Supporting the service manager, contributing to health and social care agenda, leading on professional, practice and service improvements to ensure safe effective person centred care delivery


It is essential you are a Mental Health Nurse (RN3/ RNMH) with current NMC registration and are educated to minimum degree level/ or equivalent. You should also have specialist knowledge and application of risk assessment/ ris k management factors impacting on metal health/ mental illness/ wellness and recovery for people with complex/ unstable care needs


The hours of this post are worked 9.00am to 5.00pm.
