CONSULTANT PHYSICIAN MEDICINE FOR THE ELDERLY

Applications are invited for a new Consultant Physician to join the Medicine for the Elderly service in Dundee and Angus. This post gives the opportunity for the successful applicant to join an enthusiastic and growing team that is expanding into the community, while continuing to provide a high quality hospital service.
Medicine for the Elderly (MFE) Services have undergone extensive re-design over the last four years. We have a needs-based policy and are able to provide care to the vast majority of frail older people admitted via Acute Medical Unit Ninewells and managed via Acute Frailty Service. From there admissions go to one of several destinations: the MFE wards in Ninewells (wards 5/6) if acutely unwell or needing extensive tests; the Acute MFE Unit for person-centred MDT assessment and discharge planning; step-down to their locality/cluster inpatient Units; discharge to MFE-supported community teams across Angus, Dundee and North East Fife.
The team has a diverse medical and nursing team including at present 6 Advanced Nurse Practitioners and a Nurse consultant in MFE. The Medical team has a strong cohort of Specialty Doctors and we have a reputation of enhancing this role and opportunities for doctors. They are also committed to expanding the number of General Practitioners with an interest in MFE.
From August 2014 we extended development of our service to include a Dundee Cluster model to enhance care for the frail population of Dundee following on from successful Angus Locality model. The Medicine for the Elderly team aligned to specific clusters of GP practices with the aim of delivering and overseeing patients care across all patient journeys for frail people. Hospital teams and wards in Ninewells and Royal Victoria Hospital are now aligned to these Localities or Clusters.
The team has a vibrant stroke model that is centralised in Ninewells Hospital for acute stroke and then ongoing care is delivered by respective consultants covering these localities. Stroke rehabilitation is delivered in Royal Victoria Rehabilitation unit in Dundee and Stracathro Stroke Rehabilitation Unit in Angus.
We have an Enhanced Community Support (ECS) team in-situ working in our communities which allows proactive multidisciplinary team assessment of frail patients by teams based around GP Practices. Respective Cluster MFE Consultants support the GPs and community MDT, culminating in regular Practice-based MDT meetings. From the winter of 2017 we have complimented our ECS model by adding a Rapid Assessment (DECSA) at home model where frail people with Medical issues can also be managed at home.
In addition our Medicine for the Elderly Team is committed to addressing the local Polypharmacy issue in frail people via a collaborative approach between Locality Pharmacists, GPs and respective MFE Locality or Cluster Consultants. A programme of collaborative reviews in Care Homes and in GP Practices has commenced.
We also deliver an intensive orthogeriatric model with MFE team reviewing all frail patients admitted as emergency to orthopaedic department, and supporting all aspects of ongoing care until patient discharged from Ninewells Hospital. This service is delivered by Specialty Doctors and closely supported by respective Cluster/ Locality MFE Consultants. In addition we have recently developed a step-down model of care for selected orthopaedic patients to be looked after by a dedicated consultant geriatrician.
The successful candidate must hold MRCP and be on the Specialist Register in Geriatric and General Medicine, or be within six months of the anticipated award of CCT at date of interview or already in possession of CESR in Geriatric and General Medicine at the time of interview for the post.
The post is offered on the basis of 10 programmed activities in the first instance. However, applications are also welcomed from candidates who for personal reasons may be interested in working part-time.
The post has an on-call expectation and successful candidate will join a 1:6 Medicine for the Elderly on-call and weekend rota covering Ninewells Acute Frailty Team.
