The purpose of this post is to provide a nurse specialist role to support a small nurse led team to engage with our most vulnerable populations and to offer Keep Well Health Checks, and health behaviour change advice and support including case management for the more complex cases. The Nurse Specialist will play a key role in the case management and will line manage the Keep Well Nurses.
The vulnerable populations include gypsy travellers, homeless, offenders/ex-offenders, substance misusers, BME community, the Deaf Community and Carers living in Lanarkshire.
The team work in partnership with a number of organisations.
The Nurse Specialist will be responsible for local action plans/evaluation and reporting on the outcomes from the programme.
