The EUPD Nurse Practitioner role has been developed to assist people with an Emotionally Unstable Personality Disorder (EUPD) to be as independent as possible in their home setting. You will work in partnership with people with emotionally unstable personality disorder (EUPD) to develop their autonomy and promote choice by ensuring they remain actively involved in finding solutions to their problems, including during crises and by encouraging them to consider the different treatment options, life choices available to them and the consequences of the choices they make.  As an integral member of the multi-disciplinary team you will work in partnership demonstrating compassionate caring behaviours to fulfill the key functions of the Community Mental Health Services in particular the post-holder will be responsible for the assessment, formulation, implementation of structured clinical care using a strength based recovery focused approach, informed by completion of the GGC relevant training as well as varied supervision. You will provide care that proactively involves the person and optimises self-management to patients with Emotionally Unstable Borderline Personality disorder.  You are required to undertake duties without direct supervision as well as supporting and supervising work undertaken by registered and unregistered staff. You are responsible for proactively promoting partnership working with patients, carers and other professionals/ agencies in line with legislative frameworks, agreed standards and clinical policies. This involves working in a variety of home/ near to home community environments. You will have a key role in providing home support which may vary depending on the support the patient requires. Communication with in-patient areas to support and facilitate discharge planning will be a key responsibility. You will participate in regular caseload line management supervision and clinical supervision are critical aspects of the role. You will support the nurse team leader in the ongoing learning and development of the staff team and the Assertive Outreach team key functions are important aspects of this role.  It is essential that you are currently a Registered Mental Health Nurse with the NMC and that you are educated to degree level or have equivalent experience. The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
