To undertake the role of the specialist nurse, utilising specialist clinical knowledge to inform decision making and clinical judgment.  To provide specialist advice to the multidisciplinary team in relation to the management of patient conditions through the assessment, treatment and review planning process in partnership with the patient / carer.

Provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives.

Support the development of the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence based standards, guidelines and policies.

The post holder will enhance patient care by being an integral part of the MDT team, supporting the medical management of patients within the scope of the nurse practitioner role and supporting the outpatient services and managing case loads.

The post holder will be required to manage respiratory patients out with the medical directorate and work with the Health and Social Care Partnership in joint working for respiratory patients.

The post holder will participate in the monitoring, planning, delivery and evaluation of patient care key to delivering criteria led discharge and reduction in length of stay, pivotal to the delivery of the of the government four hour standard as well as being a key driver in anticipatory care planning for high risk respiratory patients.

This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
