ICASS (Integrated Community Assessment & Support Service) has the overall aim of improving the quality of care and outcomes for older people with the intention of allowing them to remain independent within their own homes or communities.
Nurse Practitioners work effectively as senior members of the Hospital at Home Team ensuring the delivery of high quality, clinically effective care, utilising clinical skills and knowledge.
We are seeking individuals to join the established Hospital at Home service within the West Division.
You will be required to deliver a high standard of safe and effective acute medical patient care within patient’s homes and must have the ability to work autonomously in this evolving service.
You should ideally have a clinical decision making qualification at degree level or above. This should include the ability to work autonomously as a Nurse Practitioner as well as having clinical skills including venepuncture, ECG, IV additives and cannulation. A non-medical prescribing qualification would be desirable but not essential; however, it would be a requirement to undertake this in the future.
Candidates who commence on band 5 will be required to successfully complete the appropriate qualification/s or training as outlined within the Annex 21 competency framework prior to progressing to band 6.
Candidates who meet the full requirements of the person specification for band 6 will commence payment at band 6 with no requirement to progress through the competency framework.
You will have good communication skills and have a flexible attitude to work. The ability to travel within the area in a timely manner is essential.
This post involves flexibility to work over a 7 day period, working shifts covering the period from 08.00 – 22.00 On appointment you will be based at either QMH or Whytemans Brae Hospital Kirkcaldy. Cross team working is an expectation of the role.
