You will work under the guidance and support of qualified Rehabilitation Enablement Services (RES) professionals as part of the multi-disciplinary team in the delivery and implementation of individual patient treatment programmes, within community settings. You will accept patients assessed by a qualified practitioner and referred by the team and carries these as own caseload and undertake direct and indirect patient care. You will carry out clerical and housekeeping duties including stock and monitoring and ordering. You will independently carry out assessment for low level equipment e.g. commode, walking stick or bed lever and provide such, based on service protocols and competency based training. You will carry out review visits and final visits for patients prior to discharge from the service within agreed parameters of the patient care plan and service protocols and ensure valid consent has been gained for the programme of care. You should have relevant experience working in a health or social care setting, preferably within community health or social care experience
You should have excellent written and verbal communication skills and have attained an SVQ Level 3 in health or social care or equivalent e.g. development and learning at HNC or related higher level qualification. You should be able to demonstrate understanding of the rehabilitation process. The shift pattern for this post may be over 7 days.
This is a Fixed term post for 12 months
