This is an exciting opportunity for a suitably qualified & experienced physiotherapist who wishes to work within community based services.
This is a full time post based within an innovative Health & Social Care Partnership (HSCP). This post incorporates 2 distinct services – one being a Community based domiciliary Physiotherapy post covering the Dumbarton locality for 2 days per week. The other part of the post involves a clinic based service for 3 days per week, which covers the Dumbarton & Helensburgh Locality.
The Community based Domiciliary post supports adults aged 16 – 64 years of age who present with complex physical disabilities. You will work within a multi-disciplinary team, will be responsible for a designated caseload, will carry out advanced assessments and formulate appropriate treatment plans which will involve joint working and onward referral to colleagues within the HSCP and will support adults to remain as active and independent in their community as possible
The clinic based service involves close collaboration with a small clinical team, working with adults aged 16 – 65 years of age who have physical disability as result of a diagnosed neurological condition. This team works within a well equipped gym/multi purpose room, offering treatment & rehabilitation sessions and goal planning in order to support adults manage their long term and often degenerative conditions. This post will involve making onward referrals to colleagues within West Dunbartonshire HSCP and NHS Highland for additional services and support.
This is an exciting opportunity for an experienced practitioner to maintain their clinical skills working with adults with a wide range of presenting conditions in addition to working within an integrated service in order to promote positive destinations for the people of West Dunbartonshire & Helensburgh.
A lunch time information session for the Clinic based service has been arranged for Tuesday 13th August (12.00pm until 1.30pm), interested candidates are welcome to attend. You should have a Degree in Physiotherapy and be currently registered with the HCPC. You should have significant NHS experience through medicine for the elderly, neurology, orthopaedics, acute care, respiratory and MSK out-patients. The shift pattern for this post is Monday - Friday 8.30am - 4.30pm.
