The older persons mental health team provides a 5 day per week service to adults over 65 years old with severe mental health problems. Within the community setting care is delivered by CPN in collaboration with the Multi-Disciplinary Team. It is based on best available evidence for practice and is planned and delivered in a manner which promotes personal choice. Accepts referrals from GP, psychiatrists and CPN colleagues. You will be responsible for specialised assessments, treatment packages and discharges of said defined caseload. You will make decisions regarding referrals to the team through the process of screening. This involves working collaboratively with medical staff and social work staff and provide specialist assessments and reports to referrers as required. You will instruct junior members of staff with regards to following devised said care plans. It is essential that you are currently a Registered Nurse with the NMC and that you have relevant experience as a Band 5. You should have specialist knowledge of mental health nursing care. The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
