The Mental Health Older adult service is establishing a Home Treatment Team. The purpose of this team is to provide additional support to patient’s known to the service to prevent inappropriate hospital admission for people with dementia; support early discharge from hospital wherever possible; maintain individuals at home where there is a risk of carer breakdown; work jointly with other statutory and non statutory services to improve patient experience and finally to work with those individuals who require specialist intervention as part of a secondary service to ensure equity of access to other services.
A key area of work for these posts will be responsiblity for the assessment of care needs and the development, implementation and evaluation of patient centred care.
NHS Borders is looking for registered nurses at band 5 who have proven clinical skills and preferably with experience caring for Older Adults with dementia to join this Team.
