Aberdeen City Health & Social Care Partnership covers the geographical area of Aberdeen City which has a population of approximately 235,000. Within Aberdeen there are currently 28 GP Practices. The City is grouped into three localities, within the three localities the general medical services are delivered in a model of four cluster groups, each with a Cluster Clinical Lead.
The Aberdeen City H&SCP is responsible for the planning and delivery of Adult Health and Social Care Services in Aberdeen City as well as hosting Older People and Rehabilitation hospital based services at Woodend Hospital and Sexual Health Services based at the Community Health and Care Village on behalf of Aberdeenshire and Moray Integrated Joint Boards.
Old Aberdeen Medical Practice has a list size of around 11000 patients. The team at Old Aberdeen consists of, 8 salaried GPs (5.5wte), a Nurse Practitioner and an experienced Practice Nursing team.
The post will be primarily based at Old Aberdeen Medical Practice. However candidates should note that they may be required to work at other practices across the city subject to clinical demand.
Aberdeen City H&SCP is currently reviewing the how we provide GMS services from our managed ‘section 2c’ practices. A consultation process is underway to review the current service provision and how this will look in the future.
While this consultation process is in its early stages this will provide the successful candidate with the opportunity to be involved in shaping the future service provision for our population as we look at innovative ways of providing sustainable, resilient and person-centred care.
