Aberdeen City Health and Social Care Partnership is delighted to be able to offer opportunities for Nurse Practitioners (NP).

These posts are predominantly for the Hospital at Home team and there may also be potential to provide NP input to other services within the Health and Social Care Partnership, including but not limited to, the Community Geriatric Nursing Service, Woodend Hospital and Community Nursing Teams.   This will provide post holders with a breadth of experience including in-patient and community settings.


Hospital at Home (H@H) is a 7-day service providing hospital level care by a Multi-disciplinary team for patients in their own homes or homely setting.  The service provides an alternative to acute hospital admission, where it is safe to do so.  The H@H team is based in Woodend Hospital and provides a City-wide service.  This is a key service supporting the Aberdeen City element of the redesign of the Frailty Pathway in NHS Grampian.


At an exciting time of change and development within Aberdeen City HSCP, successful candidates will have the opportunity to shape the H@H service and enhance their own personal & professional development across the breadth of unscheduled care in the community.


We are looking for Registered Nurses who hold a PGCert in Advanced Clinical Practice.  Preferably you will have both acute hospital care and community experience.


For interested parties who do not possess the relevant qualification, consideration will be given to  

  • applications from Band 5 nurses with AfC Annexe 21 being applied 
  • applications from existing Band 6's who do not hold the relevant qualification will be offered the post on an 18 month secondment basis to allow them to gain the qualification, thereafter substantive NP post would be re-advertised.
