Job Purpose


The Community Links Workers (CLWs) Co-ordinator will provide management, support and leadership to the CLWs who are aligned to GP Practices throughout Lanarkshire.


Additional Information Specific to the Role


The CLWs Co-ordinator will provide co-ordination for the service and direct line management to the CLWs. The CLWs are non-clinical practitioner based in or aligned to a GP practice or cluster/locality who will work directly with patients to help them navigate and engage with wider services.  They offer non clinical support to patients, enabling them to set goals and overcome barriers, in order that they can take greater control of their health and well-being.  They support patients, enabling them to identify and access relevant resources or services in their community.  They will also map local services, engaging with and developing productive relationships with these services including keeping informed of the status of existing and new services. 


To be successful the co-ordinator will be required to work closely with different stakeholders to ascertain development needs and aspirations and to co-create meaningful and effective solutions that maximise their contribution in a community orientated approach to primary care.
