You will be expected to provide an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies. You will support the District Nurses and colleagues in delivering care to patients within the caseload.  You will be responsible for delivering care to patients as delegated by the district nursing sister/charge nurse.  The caseload population is derived from either the GP practice to which the team are aligned, or within a defined geographical population and/or a defined client group.  The caseload comprises of patients with a range of complex needs requiring high-level nursing interventions, such as cancer and non-cancer palliative care patients, those requiring nursing care in relation to chronic disease management and the provision of skilled technical interventions, e.g. subcutaneous infusions, central venous lines and tracheotomy management. It is essential that you are currently a Registered Nurse with the NMC and that you have evidence of continuing professional development.  The shift pattern for this post is between Monday – Sunday (including weekends and public holidays) 4 7.5 hour shifts.
