Hours: 18.75hrs per week. This is a fixed term post until 31st March 2022.

An opportunity has arisen for a Band 6 registered Nurse to join our small but dynamic team in Midlothian.
The role involves working with people most vulnerable to poor health and health inequalities; to support them to improve and manage their health and to access local services, such as drug treatment or welfare rights. Nurses provide an outreach service and offer time-limited self-management support. This service aims to improve the quality of life of people with one or more long-term health conditions and/or a significant difficulty in their life.
