An exciting opportunity has become available for an individual with substantial clinical and leadership skills to join the ACE Practitioner team based within the Combined Assessment Unit at Crosshouse Hospital. This service works in close partnership with South, North & East Ayrshire Health and Social Care Partnerships and Acute Care Directorate.
You will provide specialised, rapid assessment and care for patients with a diverse range of complex presentations and multiple co-morbidities. Supporting the delivery of the acute frailty pathway, prevention of inappropriate admission to hospital and facilitation of early supported discharge.
You should be able to demonstrate; competencies for care and rehabilitation of people with frail/complex needs, excellent communication skills and knowledge of NHS and Social care pathways. Ensuring fail older people have the opportunity to return to their own homes with community-based management where appropriate and avoid unnecessary or lengthy acute hospital admissions.
You require to be educated to Degree level with DIP/BSc with current Professional registration or equivalent, with relevant post-graduate registration qualification or experience in older people’s assessment/ community/ rehabilitation.
This is a fixed-term post for 6 months.
Hours: 37.5 per week.
Full driving licence is essential.
