The frailty team has been successfully working for the last 2 years to improve the journey for frail patients in DGRI. We are excited to be able to advertise the frailty nurse on a permanent basis. The frailty nurse has been pivotal in improving the flow of frail patients with DGRI.
As part of a multidisciplinary team the post holder will support in the delivery of the Comprehensive Geriatric Assessment (CGA) to older adults on admission to DGRI. The role will facilitate CGA within the combined assessment unit of DGRI and provide an in reach support throughout the hospital. This role will support appropriate, safe onward transfer and supported discharge of frail elderly patients through appropriate screening and patient/family/carer involvement.
The post holders will act as an expert resource providing advice where required to members of wider multidisciplinary team. This role will incorporate audit to measure impact of service and identify ongoing areas for improvement. As part of the wider team the post holder will contribute to strategic issues both locally and nationally.
