Band 4 – Carer Information Strategy Worker

The role is to assist in co-ordinating communication and information sharing across stakeholders such as GP practices, health and social services and third sector services to support Carers.   You will support the carer’s service by developing robust mechanisms for access to information and training which equips carers with the necessary knowledge, skills and confidence.  You will create and build on information and communication systems to enable carers to support the person they look after and in effect assist them to self-manage their condition. The provision of supported self-care is a key underpinning holistic approach to care which involves the encouragement of a greater partnership between HSCP, community partners, carers and the voluntary sector.  You will develop robust structures and protocols within a primary care setting to identifying carers, assess carers information, training and support needs both individually and as a community across the HSCP

Work within NW Locality of Glasgow City Health and Social Care Partnership.

Improve carer awareness amongst NHS Primary Care staff and deliver on the ‘Better Health, Better Care in Scotland’ policy.   You will link with acute workers to support the focus on increasing referrals from acute services / discharge managers etc.   You will support test for change acute carers referral developments and provide reference screening point for Ski-gateway carers referrals, signpost onto local carers services.  You will become a member of the Carers Advice and Information Team (Cait) developing, promoting online / social media carers awareness as part of GC Carers partnership.  You should have experience of working with carers within health and or social care is desirable.   Experience of team working with excellent communication skills.  The shift pattern for this post is Monday – Friday 9.00am – 5.00pm.
