This post offers an exciting opportunity for an ambitious and committed Consultant in Geriatric Medicine to develop innovative, holistic, multidisciplinary services for older people in Ayrshire with a focus on frailty assessment and early intervention.  

The post will continue to build on a successful pilot project within NHS Ayrshire and Arran involving the Frail and Older People multi-disciplinary team and redesign of the patient journey, focusing on early assessment and intervention that is centred on the patient and based around acute comprehensive geriatric assessment. This is known as the Frailty Pathway. Development of the multi-disciplinary Frailty Team will support this new way of working that and challenge existing pathways of care. The aim is to transform the delivery of emergency care of people admitted with frailty related conditions, offering real alternatives to acute hospital admission and better support them either at home or in a homely setting. This aligns with the Scottish Governments 2020 Vision. Five Acute Care of the Elderly (ACE) Practitioners are already in post and working alongside a Consultant Geriatrician.

The post will be based at University Hospital Crosshouse, working within our Combined Assessment Unit, and supporting other front door and inpatient areas within our acute and community hospitals. Close working with Community Services within our Health and Social Care Partnership frameworks to support our new Acute Frailty Pathway development will be an integral component of the role. There will also be the opportunity to support early discharge and find realistic alternatives to acute hospital admission. There is also the opportunity to support patient care within our community hospital at Woodland View, Irvine and develop any specialist interest that the candidate may bring.
The post holder will have the opportunity to develop a key leadership role in further integration of health and social care with a focus on prevention, anticipation and supported self-management. When hospital treatment is required, the focus will be on ensuring that people get back to their home or community environment as soon as appropriate, with support that minimises risk of re-admission. Whatever the setting, care will be provided to the highest standards of quality and safety, with all decisions being person-centred. 
The post holder will also provide ongoing leadership and training in the expansion of innovative training posts that are community facing which offer exciting opportunities for doctors within the GP training programme and Clinical Development Fellows to work at the interface between Acute, Community and Primary Care Services, within a multi-disciplinary team.

NHS Ayrshire and Arran have built a new and supportive development programme for new consultants which provides additional management training to those in their first Consultant post.  There is also a strong focus on staff wellbeing within the Board. The terms and conditions of service are those determined by the New Consultant Contract (Scotland) 2004 and the post is offered on a 10 Programmed Activity basis to include a minimum of 2 SPA  The job plan will include dedicated time to recognise the additional responsibility of partnership working and service development.  Applications from individuals who may wish to work on a part-time or job share basis will be considered. Opportunities may exist for Extra Programmed Activities to be undertaken, subject to service requirements, and in accordance with national terms and conditions of service.

Accredited in Geriatric Medicine, you will have the necessary training and expertise to provide a range expertise for older patients in Ayrshire to meet the needs of these new roles. Candidates must be fully registered with a Licence to Practice and must be on the Specialist Register, or be within 6 months of the anticipated award of a CCT or CESR at the time of interview.
