As part of a significant service redesign, NHS Forth Valley, together with Stirling and Clackmannan and Falkirk Health and Social Care Partnerships (H&SCP), are developing a multi-professional workforce model of care to create and implement a Hospital at Home Service. We are looking for a dynamic and innovative leader who is also a highly experienced Advanced Nurse Practitioner to lead this service. Applicants should have advanced practice experience in acute care and have gained experience in home visiting, care homes or community hospitals. 
Evidence of leading change and commitment to developing others will be the key qualities required to implement change and develop the anticipated clinical pathways. The applicant must demonstrate outstanding leadership skills to embrace the demands of this dynamic and challenging role. The senior ANP will also provide professional and clinical supervision to our trainee Advanced Practitioners and should have experience and evidence of commitment to developing others.
The post holder must have knowledge and experience of managing undifferentiated complex care presentations. This may include managing acute presentations, exacerbations of long-term conditions, undertaking comprehensive geriatric assessments and undertaking frailty screening. The senior ANP will provide autonomous and expert assessment, diagnosis and treatment to patients and also provide education, support and clinical supervision in the development of trainee Advanced Practice staff. 
Essential Credentials for band 8a:

  • Significant Experience of managing complex care presentations including home visiting, care homes or community hospitals 
  • HEI accredited Independent Prescribing Qualification - V300
  • HEI accredited Clinical Examination & Advanced History Taking course
  • HEI accredited clinical decision-making module or equivalent practice-based competencies / experience
  • Highly developed leadership skills
  • Evidence of commitment to developing others
