A 12 month fixed term opportunity has arisen for a Service Manager to support the work of Midlothian Health & Social Care Partnership

  • To operationally lead and manage the NHS and Social Care Intermediate Care Services within Midlothian H&SCP, including responsibility for the integrated team and budgets, as well as ensuring efficiency, productivity and performance targets are achieved.
  • To ensure professional and service standards are implemented and integral to ways of working within an integrated service.
  • To provide operational, clinical and professional leadership in the running of the integrated service and influence the future delivery and clinical strategy.
  • Be accountable for the delivery and standards of care provided to patients in areas of responsibility, ensuring safe and effective, person-centred care, compatible with professional and national clinical standards.
  • Leadership of the delivery of the integrated quality agenda and the development of team members and practice.
  • Lead and manage the ongoing implementation of the integration of community Health and Social Care Services within the Partnership, managing employees and resources within Midlothian Council and NHS Lothian, leading on the prevention of admission and the facilitation of safe discharge, working in partnership with Acute and third sector colleagues to deliver comprehensive and seamless models of intermediate care.
