Full-Time 37.5 hours per week
Applications are invited for a 37.5 hour’s permanent Band 7 Specialist Community Therapist based at the Royal Northern Infirmary working within the Enhanced Community Service.
This exciting opportunity to work within an established team of AHPs in a community based setting. The focus for this post is provision of short term interventions to support people to remain at home when they are experiencing a recent change in their circumstances. The post holder will also work with secondary care to facilitate earlier hospital discharges for people who require increased support and rehabilitation on their initial discharge form hospital.
You will take responsibility for a community clinical caseload incorporating patient assessment, diagnosis, treatment and discharge from care. You will work within multi-professional teams to ensure co-ordinated care for patients.
You will be expected to act independently to provide a full comprehensive specialist service of assessment, diagnosis, treatment/appropriate management, onward referral and discharge to a complex range of patients referred to the service. You will have worked within a community team and have proven leadership expereince.
You will attend MDT huddles, supervise AHPs, Assistant Practitioners and be expected to deputise for the team lead in meetings related to specialist clinical area.
You will be expected to travel efficiently and effectively between various work locations and patient residences, so a full current driving license is essential.
